HOA Membership & Directory Information Form

I (we) the undersigned, residents of Schalamar Creek Country Club & Golf Community, hereby consent to become members of the Schalamar Creek Homeowner's Association, Inc., in accordance with sections 723.05 and 732.078 Florida Statutes. I (we) further indicate that I (we) waive the right to receive notice of any HOA meeting by mail, per Florida Statute 732.078.

Annual dues are $10 per household (make checks payable to Schlamar Creek HOA)
